Eligibility Requirements:

• At least 18 years of age.
• Primary diagnosis of a thought disorder, mood disorder, or anxiety disorder.
• No primary diagnosis of personality disorder, eating disorder, developmental disorder, or significant intellectual impairment.
• No medical condition that requires home healthcare/hospice care.

• If substance abuse is identified as a treatment issue, the applicant must be willing to participate in community based substance abuse services as recommended.

• Applicants with a recent history of suicide attempts, self-injury, and/or aggression will be evaluated on a case-by-case basis.

• Must have a desire for increased independence.

2: Meet with the Clinical and Administrative Team for a final review of eligibility for admission.

3. Complete all required clinical, finanical, and healthcare documents prior to admission.

Financial Eligibility :

Applicants approved for admission by the Clinical Team must also satsify one of the following finanical criteria. All other individuals will be admitted on a case-by-case basis.

- Determined to be disabled by the Social Security Administration and meet the Virginia Department of Medicaid's resource and income requirements;

- Referred from an agency which has entered into a service agreement with Gateway; or

- Have sufficient funds to pay the full fee for services at each level of the program.
